PERMIT APPLICATION REQUIREMENTS:
Every contractor is required to register with Bethel Township per Ordinance No. 164. The license is valid for one (1) year and must be submitted with a Certificate of Liability Insurance listing Bethel Township as the policy holder. The Workers' Compensation Affidavit page only needs to be submitted if workers' compensation is not listed on the liability.
